Does it come with the hardware to mount the rotopax mount onto it?

No. It only comes with the racks and strap to mount the racks to the tire. No hardware for mounting any accessories to the racks is included.

Can you mount a pair of Maxtrax on one side and a Rotopax on the other side?

You can but you would need to find a way to mount them to the metal plate has many hole to mount different things so you would just need the way to mount I have rotopax on one side shovel and axle on the other

Will this damage the spare tire?

I have had it on for two trips totaling 22 days and no damage whatsoever. I did secure it with a bicycle lock for some security to the spare.
