Will this cause ammonia or nitrate spike since they dissolve quickly?

I've never had a problem with an ammonia or nitrite spike. You obviously want to bury it in the substrate.

What are the nutrients derived from? This should be present on the label but is not. What is the chloride content?

The "vital nutrients" are clearly and unmistakably displayed on the front of the package. There is no chlorine content unless some of the minerals come in chloride form which would not be toxic. If you are looking for something like 'protein or cab' contents, you might want to read up on what 'fertilizer' is and how it works. These tabs are minerals that plant make proteins and carb from.

I have one Amazon Sword plant and two Anubias Nana plants in a 55 gallon aquarium can I space the cubes much farther apart into the gravel?

Amazon swords are heavy root feeders and should have a root tab or two right under or next to each plant. Make sure to plant swords with the top of the root crown exposed; they have a small rhizome there that needs water circulation. Anubias are epiphytes and water column feeders which need to have their rhizome exposed to water or it will rot. If you've buried yours in the substrate, you need to dig them up or they'll die.
