Just wanted to be the first to post here. This book is eagerly awaited. John Reeve has posted some amazing articles on his website and LinkedIn. Every Reliability Engineer working with RCM and/or Maximo should be considering this book. I predict a large number of 5-star reviews!

In Failure Modes to Failure Codes, John Reeve and Derek Burley address an important source of frustration in organizations failing to achieve success in their asset management and reliability programs. Central is the assumption that a CMMS out of the box will deliver reliability results. Reeve and Burley advocate for thoughtful CMMS configuration and describe how to improve overall CMMS data quality. The main focus of the book is the opportunity to dramatically improve the operation and reliability of assets through appropriately managed and applied failure analysis, built on the foundation of better failure data tied to failure modes.We wholeheartedly agree with Reeve and Burley in their central thesis that asset strategies can be better determined through a formal failure analysis: “The goal is to link every maintenance strategy to a failure mode that you are trying to prevent” (p. viii.) Chapter three, which explains RCM and failure modes with insights from an RCM facilitator is particularly valuable for understanding how a CMMS needs to be configured to support RCM and failure mode management.However, in the prescriptions for asset strategies (or decisions) based on failure analysis, there is potential for some confusion around setting priorities. While there is an appropriate discussion of Pareto analysis and addressing bad actors, the question of a bad actor’s criticality and actual risk posed to business is somewhat overlooked. Importantly, they do state that a criticality analysis should precede RCM/PMO analysis, something many organizations get in the wrong order at great expense. The addition of a more complete description of how understanding criticality and risk informs the development of asset strategies based on the results of chronic failure analysis, would round out this useful book.

Excellent book that meets a real gap in the market right now

This is a well researched and well-written book that fits a very specific gap in the market, specifically how to turn the theory of failure analysis into the reality of everyday reliability improvement operations. This is done mainly through the clever configuration of the CMMS software to capture the right data and utilize it during the regular review of an Asset Offender report for your site. The main concept of the book is the importance of systematic chronic failure analysis to the continual optimization of the maintenance strategy.The ah-ha moment in the book for me was the realization that failure information needs to be captured at component as well as asset level as the component level is where the functional failures manifest themselves and is where maintenance and repair activities mainly focused.It's not particularly long book and at just under 100 pages it won't take long to get through it. That's a good thing so don't let it put you off, my copy is already well-thumbed. It's jam-packed with practical and actionable information and ideas with neat little illustrations on nearly every page that do a great job of articulating the ideas and concepts and CMMS configuration options. A great book and well worth investing in.
