How do you get the hot therapy as this only shows how to use cold by freezing packs?

The ice/heat packs can be easily removed. For the heating therapy I put them in the microwave.

Will this work for icing both the back and right side of my left knee simultaneously?

Yes you can use this it basically will wrap ice around the entire knee. I have this one because it runs a little bit small and I use it right below the knee around the calf. I use a different one to go around my knee because my knee is a little bit larger due to multiple surgeries and what I'm trying to recover from. so this one runs a little bit on the smaller side it's probably more designed for a woman but a small to average size man could use it very effectively as well. I've used it on my knee in the pinch but I prefer the one that's a little bit larger.

Says it fits up to 21" knee circumference. Would this fit me if Just above my knee, I guess would be considered lower portion of my thigh is 21"?

All I can say is that above my knee is just under 20", fits fine. Good luck.
