Not surprisingly, it gets warm after using it for a while, and I either turn over the pillow or just pull it out. I use it with about 3 pillowcases over the top so it does not feel so freezing cold when taken out of the fridge. The gel seems to be in 4 compartments which are connected, so it is better to keep the pad lying flat when in the fridge, or flat folded in half.I got the pad because I tend to get a hot flash when first lying down to sleep. It feels great when I first get into bed. However, it seems I'm usually waking up a few hours later and it's no longer cool. If I take it out, it cools off some, but not enough for me . So my verdict- stars out great but doesn't last long. I also noticed at the time of this review, it has gone up in price substantially.
