Would this work for wax-based colored pencils?

Yes, it works on wax-based pencils. This blender marker gives a different result than Prismacolor's blender pencil, just like burnishing, oil and white pencil blending give different results. This marker is closer to the result you'd get blending with mineral oil as opposed to the blender pencil which is closer to the result from burnishing.

The prismacolor colorless blender marker PB121 has brushes on either end, no wedge. Other than that is it the same product?

No, this one has a point on one end and a wedge on the other.

Is this the same as a transfer pen?

This is NOT for alcohol markers, it is for colored pencil

will this work on a brush pen

Well it is supposed to work on markers so it probably would. But beware. I ordered two and either they were old and the alchohol had evaporated OR they DO NOT WORK AS ADVERTISED.
