The small screw in the middle of the bar came out. The bar cannot expand or retract without slipping.

That is one of my issues too! I removed bar from brackets and rotated ends in opposite directions shortening the bar until I saw a corresponding hole on the inner bar. I still had the original screw but think any properly sized screw (set screw maybe better option) would work. Then twist ends to lengthen bar. I searched instruction sheet and there is no mention of this screw.

Why do the directions say this is not to be used for chin ups and only for situps and crunches?

Bar comes with two mounting systems one way for chin-ups one for sit-ups

Can weights be attached as on a barbell?

I wouldn't do that. They could fall off and smash your foot.

Can I close the door after installing the brackets?

If you instal it on the outside of the door jam it does not interfere with the closing of the door.
