I have been fighting neck pain relating at least in part to the weight of my Nikon camera and lenses, carried on the standard neck strap. I knew I had to make a change, but I was skeptical about how this would work until I tried it. The camera is very securely attached and does not swing around freely; you can hardly feel the weight of it. In addition, the underarm/shoulder strap feature of this particular product is more comfortable and secure than an ordinary shoulder strap would be. My neck bears no part of the weight, and I am able to maneuver more freely. Picture number 4 should show the connection point for the underarm strap as connecting in the back, which is the way mine is. I saw that someone else commented on this very subject! Now for the minuses. First, there's no way I'm trusting my $1000 camera to a cheap pot metal hook that looks like it might last a week! I just don't have the money for a new camera. Second, the idiot that designed it put the adjuster right in the middle of your back where no one built like a human being can reach it! I guess you're just supposed to have a friend stand behind you when you want to take photos. For the price, I expected the cheap hardware, but didn't really expect it would be necessary to rip the whole thing apart and rebuild it. Oh well, for a few more bucks for some stainless steel, and a couple of hours with scissors and sewing machine, I expect to have a very nice camera strap.
