For the 5-Speaker configuration, the answer is no. The plate cutout is taller than a standard square wall switch plate. So it wouldn't work with a two switch cover plate from the hardware store.

Q: Do I need to use a Low Voltage 1 gang wall mount for this?

You could mount it direct to the wall of you create a hole for the back end, but I prefer to use the low voltage wall mounts. Use an "old work" box if putting it on existing drywall

Q: Can Banana plugs be used on both sides? or does one side have to be 'wired'. Want to make sure a plug on one side won't prevent a plug on the other.

In short, yes. You can wire or plug on either side, wire on both or plug on both.

Q: Dose this 5 speaker wall plate fit a single electric gang box? If not what type of box is needed?

Fits single gang...although 1 screw on my mine did not line up with the gang box screw hole. Mine was hidden behind a tv, so I didn't really care. I screwed the top screw right into the drywall above the gang box.
